- Guessing or option-selection issue: The student eliminated choices unreliably, changed a correct answer without evidence, or selected an answer based on an unsupported cue.
- Review-process issue: The student had the information needed but did not check a relevant step, estimate, reread, or verify the answer in a useful way.
Ask students to select one primary error type and, only if needed, one secondary type. If every error receives four labels, the log becomes less useful. The aim is not to capture every possible cause; it is to identify the most teachable cause of the next practice decision.
Add confidence to distinguish knowledge from calibration
Confidence is a valuable field because correct and incorrect answers do not tell the whole story. A high-confidence error often deserves prompt attention: the student may hold a misconception, use an unreliable shortcut, or overlook a recurring condition. A low-confidence correct answer may indicate that the student needs more fluency, a clearer method, or better evidence for trusting the answer.

Second, low-confidence correct responses should lead to short practice that asks students to explain the method, not merely repeat the answer.
Write an error analysis that leads to action
A strong entry contains enough detail to change behaviour. Compare these two notes:
Weak note: “Read more carefully.”
Useful note: “I chose an answer that matched one sentence but did not test whether it answered the whole question. Next time, I will restate the question in my own words before comparing choices.”
The useful note identifies the decision that failed and the replacement action. It is brief, observable, and testable on the next question.
Teachers can support this by asking students to complete a consistent sentence stem: I missed this because ____. Next time, before I choose an answer, I will ____. For mathematical or multi-step work, a helpful variation is: The first step where my work changed course was ____. A better first check would be ____.
Schedule review instead of relying on one correction session
Review should not end when the student reads the explanation. Build a small schedule into the log. The exact timing can be adapted to the student’s workload, but the structure should include an immediate correction, a near-term check, and a later retrieval attempt.
- Same day: Diagnose the error, complete the corrected approach, and work through one comparable example with support if needed.
- Within the next week: Attempt a small set of similar questions without looking at the original explanation. Record accuracy and confidence again.
- Later cumulative review: Include one or two questions from the same category alongside other skills. This tests whether the revised method is becoming durable and usable under mixed conditions.
If the same error appears again, do not simply add another identical entry. Mark it as recurring and adjust the response. A recurring knowledge gap may need direct instruction or a different explanation. A recurring timing problem may need a pacing routine. A recurring misread may need a visible annotation or checklist habit.
Choose the next questions deliberately
The “next questions” field prevents the common response of assigning more of everything. Choose questions based on the diagnosed error, not just the broad subject area.

Keep the next assignment small enough to review well. Five targeted questions with written reasoning can be more informative than twenty questions completed without diagnosis.
Use the log in teaching conferences and independent study
A teacher or tutor does not need to inspect every line. Instead, review the log at regular checkpoints and ask: Which two categories appear most often? Which are high-confidence errors? Which entries have been retested? What is the next smallest intervention?
